Lixisenatide, marketed as Adlyxin in the United States and Lyxumia elsewhere, is a once-daily GLP-1 receptor agonist derived from the same exendin-4 scaffold as exenatide. It is characterized as a short-acting or prandial agent, and that classification — rather than its dosing frequency — is what determines how it behaves.

What is lixisenatide?
Lixisenatide is a synthetic peptide based on exendin-4, modified with additional lysine residues at the C-terminus. It is administered once daily by subcutaneous injection, typically before a meal.
Despite once-daily dosing, it is pharmacologically a short-acting agent. Its plasma concentration rises and falls within the day rather than maintaining continuous receptor occupancy. That transient exposure profile is the source of its distinctive clinical behavior.
How lixisenatide works
Lixisenatide activates the GLP-1 receptor with the usual downstream consequences, but the balance among them is shifted. Because exposure is intermittent rather than continuous, its effect on gastric emptying does not undergo the tachyphylaxis that develops with long-acting agents.
The result is an agent weighted toward post-prandial glucose control. It blunts the rise after eating more than it lowers the fasting glucose. Long-acting agents do the reverse. This distinction is the reason lixisenatide was paired with basal insulin: basal insulin handles fasting glucose well and post-meal excursions poorly, so a prandial-weighted GLP-1 covers the complementary part of the curve. That logic underlies the fixed-ratio combination product pairing lixisenatide with insulin glargine.
What the evidence shows
Lixisenatide's glycemic effect is modest relative to the modern agents, with HbA1c reductions and weight changes at the lower end of the class.
Its historically important trial is ELIXA, the first cardiovascular outcomes trial completed for a GLP-1 receptor agonist. Conducted in patients with type 2 diabetes and a recent acute coronary syndrome, ELIXA found lixisenatide neutral for major adverse cardiovascular events — safe, but without benefit.
ELIXA's importance is contextual. Together with the neutral EXSCEL result for exenatide and the positive results for other agents, it establishes that cardiovascular benefit within this class is agent-specific and trial-specific. It cannot be extrapolated from one molecule to another, and clinicians who treat GLP-1 agents as a single interchangeable category get this wrong.
Safety and current role
The adverse effect profile is the class pattern, with nausea prominent given the pronounced gastric-emptying effect. Hypoglycemia risk is materially increased in combination with insulin or a sulfonylurea, which is directly relevant given its use alongside basal insulin.
Lixisenatide has largely been displaced in practice by more potent agents, and its commercial availability has narrowed in some markets — current supply should be verified rather than assumed. Its enduring clinical lesson is the prandial-versus-basal distinction, which remains useful whenever a patient's problem is specifically the post-meal excursion rather than the fasting number.
