Dulaglutide, marketed as Trulicity, is an FDA-approved GLP-1 receptor agonist given once weekly by subcutaneous injection. It sits in the same class as semaglutide but occupies a different regulatory position: it is approved for glycemic control in type 2 diabetes and to reduce cardiovascular risk in specific populations, and it is not approved for chronic weight management.

What is dulaglutide?
Dulaglutide is a long-acting GLP-1 receptor agonist built as a fusion protein: two GLP-1 analog peptide chains linked to a modified human immunoglobulin Fc fragment. That Fc component is the engineering trick behind its duration. It slows renal clearance and protects the molecule from rapid enzymatic breakdown, producing a half-life long enough to support once-weekly dosing.
It was among the agents that established weekly injection as an ordinary expectation in diabetes care rather than a novelty. For patients, the practical appeal is fewer injections; for clinicians, the relevant fact is that a weekly agent takes several weeks to reach steady state, so both benefit and side effects settle on a slower timeline than a daily drug.
How dulaglutide works
Like the rest of the class, dulaglutide activates the GLP-1 receptor. The downstream effects are glucose-dependent insulin secretion, suppression of inappropriate glucagon release, slowed gastric emptying, and central satiety signaling.
The glucose-dependent quality of the insulin response is the clinically important part: because the effect scales with the prevailing glucose level, GLP-1 agonists carry a low intrinsic hypoglycemia risk when used alone. That risk rises considerably when they are combined with insulin or a sulfonylurea, which is where dose reduction of the background agent becomes the relevant conversation.
What the evidence shows
Dulaglutide was evaluated across the AWARD program, which established its glycemic efficacy against a range of comparators including metformin, sitagliptin, insulin glargine, and other GLP-1 agents.
The trial that distinguishes it is REWIND, a large cardiovascular outcomes trial. REWIND is notable for its population: it enrolled a majority of participants without established cardiovascular disease, studying primary as well as secondary prevention, and it ran over a long follow-up period. It demonstrated a reduction in major adverse cardiovascular events. That evidence supports its cardiovascular indication and is a genuine point of differentiation within the class.
On weight, dulaglutide produces reductions that are real but modest — consistently smaller than what semaglutide or tirzepatide achieve at their weight-management doses. This is the honest answer to the patient who has heard GLP-1 agents described interchangeably as weight-loss drugs.
Safety and labeling considerations
The adverse effect profile is the familiar class pattern: gastrointestinal symptoms — nausea, vomiting, diarrhea, abdominal discomfort — concentrated around initiation and dose escalation, and usually manageable by holding or stepping back rather than discontinuing.
Labeling carries a boxed warning regarding thyroid C-cell tumors based on rodent findings, with contraindication in patients who have a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia type 2. Pancreatitis, gallbladder events, and acute kidney injury in the setting of volume depletion from GI losses are described in labeling. Delayed gastric emptying is relevant to the perioperative and anesthesia setting.
As with the whole class, patients using insulin or sulfonylureas need those agents reviewed to manage hypoglycemia risk.
Dulaglutide compared with the rest of the class
Against semaglutide, dulaglutide is generally the weaker agent for both glycemic lowering at higher doses and for weight reduction, while offering cardiovascular outcome evidence in a broader primary-prevention population. Against tirzepatide, which engages both GIP and GLP-1 receptors, the gap on weight is wider still.
The framing that serves patients best is that these are not interchangeable products with a shared effect size. They differ in potency, in approved indication, and in what has actually been demonstrated in outcome trials.
