Spock eyebrows, also called Spock brow, Mephisto brow or Joker eyebrows, are the exaggerated, upward-arching outer eyebrows that sometimes appear one to two weeks after Botox® in the forehead or frown lines. They are not an allergic reaction and not a sign the product was bad. They are a dosing-and-placement problem: the outer part of the forehead muscle kept working while the rest was relaxed, so it pulls the tail of the brow up.
The good news is that Spock brow is one of the easiest Botox complications to correct. A small touch-up of one to three units into the overactive area usually settles the arch within a week. Here is why it happens, how injectors prevent it, and what to do if you see it in the mirror.
¿Qué son las cejas de Spock?
Your eyebrows are held in balance by two sets of muscles. The frontalis is the broad muscle of the forehead; it is the only muscle that lifts the brows. Working against it are the brow depressors: the procerus between the eyebrows, the corrugators above the inner brow, the depressor supercilii and the upper part of the orbicularis oculi around the eye.
When an injector treats horizontal forehead lines, the goal is to relax the frontalis evenly. If the central and medial forehead receive adequate dosing but the lateral (outer) frontalis above the tail of the brow is left untreated or under-treated, that outer strip keeps contracting. With nothing holding the center of the brow up and nothing holding the outer brow down, the lateral brow peaks sharply. The result is the quizzical, surprised arch that gives the complication its name.
The same thing can happen after glabellar (frown-line) treatment alone: relaxing the central depressors lets the brow center drop slightly while the lateral frontalis is still fully active, which makes the outer arch look higher by comparison.
How Soon Does Spock Brow Appear, and How Long Does It Last?
Botulinum toxin starts working within three to five days and reaches full effect at about two weeks. Spock brow usually becomes noticeable in that window. If nothing is done, it persists for as long as the treatment lasts, typically three to four months, fading gradually as the medial forehead regains movement. In practice, almost no one needs to wait that long, because correction is simple.
Cómo prevenir las cejas de Spock causadas por Botox
Prevention is the injector's job and comes down to assessment, mapping and conservative dosing:
- Assess the brow in motion before injecting. Asking the patient to raise their eyebrows reveals how far laterally the frontalis is active and where the natural brow peak sits. Patients whose frontalis extends far toward the temple need lateral coverage.
- Treat the whole active frontalis, not just the lines. Placing points only where the wrinkles are deepest, usually the center, is the classic setup for Spock brow. The lateral forehead often needs one or two small-dose points above the brow tail.
- Stay high enough above the brow. Lateral points placed too low can weaken the brow lifter and cause brow droop instead. Experienced injectors keep lateral frontalis points well above the orbital rim and keep doses there small.
- Treat the frontalis and the glabella together when both are indicated, so lifters and depressors are relaxed in balance.
- Dose conservatively and schedule a two-week check. It is easy to add a unit or two at a follow-up; it is impossible to take units out. Many injectors deliberately under-treat the lateral forehead on a first visit and refine at two weeks.
The injection points involved are covered in our guide to forehead Botox injection sites and in the broader Botox face chart.

Can Spock Eyebrows Be Fixed?
Yes, and usually quickly. The fix is to relax the overactive lateral frontalis: the injector places a very small dose, commonly one to three units per side, directly over the point where the brow peaks, keeping the needle superficial and well above the brow. The arch typically softens within four to seven days as that strip of muscle relaxes to match the rest of the forehead.
Two cautions. First, wait until the original treatment has taken full effect, about two weeks, before correcting; treating at day four can lead to over-correction once the first dose finishes kicking in. Second, the touch-up should be modest. Too many units placed too low will trade an arched brow for a heavy, flattened one, which is harder to live with.
Most providers correct Spock brow at no charge when it follows their own treatment. If you are not comfortable returning, any experienced injector can assess and correct it.
Spock Brow vs. Other Brow Problems After Botox
- Brow ptosis (heavy, drooping brows) is the opposite problem: too much frontalis relaxation, especially low on the forehead, so the brows sag. It cannot be reversed with more toxin and resolves only as the product wears off.
- Eyelid ptosis is a drooping upper eyelid caused by toxin reaching the muscle that lifts the lid. It is a different complication, covered in our article on ptosis after Botox.
- Asymmetry where one brow sits higher than the other is usually a dosing imbalance between sides and is corrected the same way as Spock brow, with a small balancing dose. See uneven results after Botox.
Preguntas frecuentes
What causes Spock eyebrows after Botox?
Under-treatment of the outer (lateral) frontalis muscle relative to the center of the forehead. The untreated outer strip keeps lifting the tail of the brow while the rest of the brow is relaxed, creating a sharp upward arch.
How long do Spock eyebrows last?
Untreated, for the life of the Botox treatment, usually three to four months. With a small corrective dose at the two-week mark, the arch typically resolves within about a week.
How many units fix Spock brow?
Usually one to three units per side placed over the peak of the arch, above the brow. The exact amount depends on the patient's anatomy and the original dose, which is why correction should be done by the treating injector or another experienced provider.
Can Dysport or Xeomin cause Spock eyebrows?
Yes. Spock brow is a placement and dosing issue, not a brand issue. Any botulinum toxin type A product, including Botox®, Dysport® and Xeomin®, can produce it if the lateral forehead is under-treated.
For Providers: Spock Brow Is an Assessment Problem
Nearly every Spock brow traces back to treating wrinkles instead of muscles: injecting where the lines are rather than mapping where the frontalis is active. Dynamic assessment, lateral coverage with small doses, and a built-in two-week review eliminate most cases.
