Masseter Botox before and after: in the first two weeks you will feel the change before you see it. Jaw clenching, grinding and jaw tension ease, but the face looks the same. Visible slimming of the lower face starts around month three, usually after a second treatment, and most of the narrowing a patient will get has accumulated by month six of consistent treatment. The dramatic photos online almost always span a year or more of maintained sessions, not one appointment.
Here is what to expect at each stage, why most before-and-after galleries exaggerate, what it means when the photos show no change, and how to judge results honestly, whether you are a patient comparing clinics or an injector building a gallery that holds up.
Masseter Botox Before and After: What You Will Actually See
- Days 1 to 7. Nothing visible. Some patients notice the jaw feels slightly tired when chewing tough food toward the end of the first week.
- Weeks 1 to 2. Clenching, grinding and morning jaw tension begin to ease. This is the result bruxism patients came for, and it arrives first. The jawline has not changed yet.
- Weeks 3 to 4. The functional effect peaks. A photo taken while clenching shows less bulging than before; at rest the difference is still subtle.
- Month 3. After a second treatment, the earliest point at which most patients can see a real change in the resting jawline. Published dosing that produces this sits at 20 to 40 units per side; lower doses often produce no visible slimming at all.
- Month 6. With treatments kept on schedule, the majority of the slimming result has accumulated. This is the honest comparison point, and the right time for a formal reassessment.
- Beyond. Maintenance rather than continued dramatic change, typically two to four sessions a year. Stop treating and the muscle gradually rebuilds over months; the effect is not permanent.
Off-label note: botulinum toxin for masseter slimming, bruxism and TMJ symptoms is an off-label use in the United States, which is one more reason the consultation, dosing and photography standards below matter.
What Masseter Botox Before and After Photos Actually Show
Start with the biology, because it sets the ceiling on what any photograph can contain.
Neurotoxin does not remove tissue. It reduces the force the masseter can generate. Held under reduced load, the muscle atrophies — slowly, and only while the load stays off. The measurable reduction in mastication force appears at two to four weeks, peaks around three weeks, and lasts roughly 8 to 12 weeks (Ghatge et al., Bioinformation, 2023). Visible narrowing of the lower face is a slower, separate process that accumulates across repeated cycles.
So a pair of photographs is showing one of three quite different things, and the honest gallery says which:
- A first treatment at two weeks. Realistically, very little. Symptom relief for a bruxism patient, yes. Visible facial change, rarely.
- Three to six months, maintained. This is where genuine narrowing begins to be photographable.
- A year or more of consistent cadence. This is where the dramatic galleries come from, and the interval is usually not stated.
A before-and-after labelled only “after masseter Botox” is missing the single most important piece of information: how long, and how many sessions. We cover the underlying schedule in how long masseter Botox lasts and how often to retreat.
Why Most Masseter Before and After Galleries Mislead
None of what follows requires bad faith. It only requires a phone, a busy clinic and no photography standard.
Clench versus rest. This is the big one. The masseter roughly doubles in apparent prominence when a patient clenches. A “before” taken with a clenched jaw and an “after” taken at rest produces a spectacular result with no treatment at all. Reputable galleries show both states, matched.
Head position. A few degrees of chin elevation tightens the submental line and narrows the apparent jaw. Rotation changes which mandibular angle you are looking at. Unless the head is in the same position in both frames, you are comparing two different faces.
Lighting. Side lighting carves shadow under the zygoma and along the mandible, which reads as definition. Flat frontal light fills it in. Switching between them between shots is the single cheapest way to fake a result.
Focal length and distance. A phone held closer distorts facial proportions. Step back and change the lens and the lower face genuinely changes shape in the image.
Everything else that happened in six months. Weight change, hairstyle, makeup, camera, and — commonly — other treatments delivered in the same period. A masseter gallery that overlaps with filler, skin tightening or weight loss is not isolating the masseter.
The discipline that solves all of this is the same one we argue for in PRP and PRF before and after: standardised photography is not a marketing asset, it is a consent instrument.
A Realistic Masseter Botox Timeline, Month by Month
What a patient should actually expect to see, assuming an adequate dose and a maintained schedule.
- Week 1 to 2. Bruxism and jaw-pain symptoms begin to ease. The jaw may feel tired on tough food. Visually, nothing. This is the point at which most patients privately conclude it did not work.
- Week 3 to 4. Functional effect peaks. A clenched-jaw photograph will show less bulging than baseline. At rest, still subtle.
- Month 3. After a second treatment, the first genuinely photographable change at rest in most patients. This is the earliest honest comparison point.
- Month 6. With consistent spacing, this is where the majority of a slimming result has accumulated and where a formal reassessment belongs.
- Beyond. Maintenance rather than continued dramatic change, typically two to four sessions a year.
The clinical version of this, with the measurement literature behind it, is in the masseter Botox results timeline.

When the Before and After Shows No Change
Two explanations account for most of it, and they have opposite fixes.
The dose was too low. Published masseter dosing clusters at 20 to 40 units per side, and doses below 20 units per side are inadequate (Ghatge et al., Bioinformation, 2023). An underdosed masseter does not produce a subtle result; it commonly produces no visible result at all. If a price seemed unusually good, this is the likely reason — see masseter Botox cost and units.
The masseter was never the problem. Lower-face width can come from the parotid gland, from skeletal width at the mandibular angle, or from buccal and submandibular fat. None of those respond to neurotoxin, and a patient treated on a wrong assumption pays in full for a photograph that looks identical. The screen for this is in masseter Botox: when the bulge is not masseter hypertrophy.
A third, less common possibility is worth knowing about: some patients develop paradoxical masseteric bulging, where the muscle appears to protrude more on clenching after treatment rather than less.
The Change Nobody Photographs
Galleries show the jawline. They do not show the two things patients most often report afterwards.
The first is functional: a jaw that tires faster on tough food for a few weeks. It is expected, it tracks the same 8 to 12 week window as the rest of the effect, and it resolves — but it is a real part of the experience and it belongs in the consultation rather than in a surprise. We answer it fully in does masseter Botox affect chewing.
The second is that for many patients the jawline was never the point. A large share of masseter treatment is done for clenching, grinding and jaw pain, where the outcome that matters is waking without a tight jaw — and no photograph captures that at all. That side is covered in Botox for TMJ.
How to Photograph Masseter Results Properly
For practices building a gallery that will survive scrutiny — and, more importantly, that will let you judge your own results honestly.
- Fix the capture standard once and never change it. Same camera, same focal length, same distance, same lighting, same background. If you change any of it, your archive stops being comparable.
- Standardise head position. Same views every time, with a repeatable reference rather than eyeballing it.
- Always shoot both states. At rest and at full clench. The clench pair is where masseter change actually shows, and shooting only one state is what makes a gallery unreadable.
- Photograph at baseline, month three and month six. Not at two weeks, which will only disappoint.
- Record what else was done. Any other treatment in the window has to be on the record, or the comparison claims something it cannot support.
- Get separate consent for imagery. Clinical record and marketing use are two different permissions, not one.

Preguntas frecuentes
How long until you see masseter Botox before and after results?
Symptom relief for clenching and jaw pain begins within one to two weeks. Visible narrowing of the lower face is slower: month three, after a second treatment, is the earliest honest comparison point, and month six is where most of a slimming result has accumulated.
Why do some masseter Botox before and after photos look so dramatic?
Usually because they span a year or more of maintained treatment rather than one session, and sometimes because the two photographs are not comparable — a clenched “before” against a resting “after,” different head position, or different lighting. Ask how long the interval was and whether both frames show the same state.
My masseter Botox before and after looks the same. Why?
The two common reasons are an inadequate dose — below 20 units per side is considered ineffective — or a lower face whose width is not coming from the masseter at all, but from the parotid gland, the bone or fat. Both are worth raising with the clinician before paying for another round.
Does masseter Botox change your face shape permanently?
No. The effect is reversible. A single treatment lasts roughly 8 to 12 weeks, and slimming is maintained only with continued treatment, commonly two to four sessions a year. Stop, and the muscle gradually rebuilds.
Should before and after photos be taken clenching or at rest?
Both, matched. Masseter change is most visible in the clenched pair, while the resting pair shows what the face looks like day to day. A gallery that only shows one state cannot be read properly.
