Terms & Conditions Your registration will not be valid and confirmed until your course fees have been received. There are no refunds
given for workshops or any other training programs unless approved by the medical director in writing. Credit will be applied to any future program. Empire Medical Training, Inc. must be notified
at least 24 hours in advance of any cancellation in order to be credited for any future programs. There are no credits or refunds for "No Shows" the day of scheduled program. Tuition fees may be
transferred to another individual, or to another activity date. Should it be necessary for Empire Medical Training, Inc. to cancel a program, a full refund of the registration fee will be refunded.
Empire Medical Training, Inc. shall not be responsible for monies registrants may have spent on non-refundable airline tickets, related travel costs, claims for lost wages, or hotel accommodations
in the event a course is cancelled or sold out. We reserve the right to refuse registration from any attendee. Seminar Location, Agendas and course instructors are subject to change. Empire
consistently adds new programs throughout the year and also deletes poorly attended workshops. Empire Medical Training, Inc. shall not be liable for any claims, losses, costs, expenses, delays or
loss or enjoyment, of any nature or kind whatsoever resulting from events beyond our reasonable control, including but not limited to acts of God, flight cancellations or changes, strikes, labor
disputes, lockouts, threats or acts of terrorism, acts of war or declared war, instructor no- shows without notice to Empire resulting in cancellation of program, hurricanes or weather conditions,
or damages to persons or property arising out of or connected to this function.
Disclaimers Enrollment in a program seminar does not guarantee that a physician is sufficiently trained
to perform a procedure. The physician will receive a certificate of achievement but this does not imply, nor is it intended to indicate any type of certification to perform a procedure. Individual
training is available with certain program topics.
